1. Bio
Waste LLC will provide a full service program including packaging
supplies, manifesting, transportation, and termination of the Facility's
infectious & hazardous waste.

A service calendar highlighting your scheduled pickups will be given
to each service location. In addition, a reminder fax will be sent
the week before on Thursday.
3.
Supplies for packaging waste will be delivered to the Facility,
as needed, including corrugated boxes, red bio hazardous bags 3.0
mil., shipment labels, and manifests at no extra charge. Sharps
containers can be supplied at a 20-25% discount for customers, we
offer 1qt, 3.3qt, 2 Gallon, & 5 Gallon (Sage & Guardian).

Boxes will be filled and sealed by the Facility's staff, including
proper labeling of boxes and completion of manifests. (One manifest
for each shipment of waste is required).
5.
Signed manifests will be returned to the Facility to serve as a
tracking document for the shipment and termination of infectious
waste generated by the Facility.
6.
Transportation will be provided in properly permitted/licensed,
refrigerated trucks from the Facility to Bio Waste LLC for termination
at approved sites according to all federal, state, and local laws.
7.
One consultation and education class per year regarding infectious
waste management will be available from Bio Waste LLC, as requested
by the Facility, at no additional cost.
8.
Bio Waste LLC personnel will serve as a liaison between the Facility
and governmental/regulatory agencies to obtain and provide accurate
and up-to-date information pertaining to applicable environmental
regulations, at no additional cost.
9.
Bio Waste LLC will maintain liability and Environmental Impairment
Insurance in excess of local and state laws for the Facility's protection.
10.
A standard two to five year Bio Waste LLC agreement will be provided
to the Facility. The Facility can determine the length of the agreement.
Price is guaranteed over course of agreement.
11.
All Bio Waste LLC billing will be mailed out monthly with original
manifests. Payment is due within 30 days of invoice date.
12.
All work is guaranteed to be completed in a professional manner.
Our workers are fully covered by Workman's Compensation Insurance.
